Eric is the Regional Director of Business Development for Monarch Healthcare Management, an organization that manages skilled nursing and assisted living facilities throughout Minnesota and Wisconsin. Eric’s main role is to coordinate with partnering hospitals to discuss ways to meet the various discharge needs of patients.

So my name is Eric Lind, and I'm the Regional Business Development Director for Monarch Healthcare Management. So, Monarch Healthcare Management, we manage skilled nursing facilities and assisted livings throughout Minnesota, and a few in Wisconsin, so we care for individuals who need help for TCU, for rehab, for long-term care, and also assisted living, just need some extra services. So I think it depends really on what I'm trying to do, so if we're trying to build referral partners, it's going into the hospitals, talking to the discharge planners that are in the hospitals about what they're really looking for and what kind of needs the community isn't meeting. In Monarch we are really big on meeting the needs that maybe aren't being met by other skilled nursing facilities. So that could be individuals that are suffering from alcohol issues, but also need rehab, maybe some drug addiction, maybe some bariatric individuals, so really going in to the social workers and saying tell me what you need me to place, and then finding the partners in the community to really develop those programs and help us out. Every single resident discharged from our facility has both, if they have home care services, we'll follow up with their home care services, make sure we're getting them in good providers. But our social services themselves are calling these individuals, asking are you thriving at home? Are you doing all right? Because there is a little known fact in Medicare. You have 30 days to come back, so if we send you home and you're not quite right, let's bring you back up and try and get you set up with the right situation, so the follow-up is the most important thing, 'cause like I tell every individual that leaves, I love that you're here, but I don't want to see you again. (laughs) For the most part, I am here. I have my phone on me at all times, I'm ready to go. It's a Monday through Friday job because for the most part I'm dealing with people that work Monday through Friday jobs, but at the same time, doing fairs and getting the word out, it's not Monday through Friday. It's the Saturday, the Sundays. It's individuals that maybe work all day and then they'll come out and do some activities on the weekends. Partnering with local senior centers, obviously they don't shut down because it's Saturday. The hours, they vary, so obviously I want to be available whenever I need to be.
